Previous Certification Reinstatement Policy for Audiology

There is a new policy for audiology reinstatement that went into effect September 1, 2009; however, this policy will be honored until December 31, 2009.

If lapsed 5 years or less—submit the following documentation:

  • Reinstatement application form (contact the ASHA Action Center at 800-498-2071 to request);
  • Reinstatement fee;
  • Evidence of professional development hours based on the number of years lapsed (i.e., 1 year=10 hours of professional development; 2 years=20 hours; 3 to 5 years=30 hours). Professional development activities must have been completed no more than 3 years prior to submission of the reinstatement application form;
  • Contact the ASHA Action Center at 800-498-2071 to request the reinstatement application.

If lapsed more than 5 years—submit the following documentation:

  • Reinstatement application form (contact the ASHA Action Center at 800-498-2071 to request);
  • Appropriate fees;
  • Confirmation of completion of a 12-week FTE clinical experience supervised by an ASHA-certified audiologist, completed no more than one year prior to the submission of the application for reinstatement;
  • Confirmation of completion of 30 hours of professional development. Professional development must have been completed no more than 3 years prior to the submission of the reinstatement application;
  • Confirmation via an official notification from the Educational Testing Service (ETS) of successful completion of the Praxis Examination in Audiology with a score of 600 or above, achieved no more than 5 years prior to submission of your reinstatement application. This requirement must have been completed before the reinstatement application is submitted.
