In this webinar, the speaker will discuss how to perform curriculum-based language assessment, beginning with the selection of areas of the curriculum to work on. Participants will explore how to use dynamic assessment to identify a student’s particular strengths and weaknesses and answer four questions based on selected curricular tasks: (1) What does the task require? (2) What does the student currently do when attempting the task? (3) What does the student need to do differently to be more successful at the task? (4) What, if any, modifications to the task are needed to support the student’s success? Differentiating apraxia of speech (AOS) from other acquired speech and language impairments is important clinically as it provides a framework for understanding the signs and symptoms of the disorder, which in turn influences which specific treatment method will be applied. However, diagnosis remains challenging since it hinges on expert-based judgment of speech features, with minimal guidance on the frequency, severity, or clustering of features required for positive diagnosis. This course provides a brief overview of the current diagnostic method and its limitations, followed by discussion and demonstration of effective objective and semi-automated speech measures for identifying acquired AOS in both stroke and progressive forms. Video case examples and demonstrations of these measures will be provided so that clinicians can immediately begin putting this knowledge into practice.

Acquired apraxia of speech (AOS) typically occurs alongside aphasia. Clinicians are often challenged with questions such as, “What should I work on first, and why/how?” and “How can I deliver intensive therapy in today’s healthcare system?” While there are a number of treatment approaches available for AOS, there are very few large-scale investigations of treatment efficacy to inform practice and limited studies that tackle the issues around intensity and concomitant disorders. This course summarizes the pros, cons, and similarities of impairment-based approaches to treatment for individuals with AOS plus aphasia. The outcomes from our latest Academy of Neurologic Communication Disorders and Sciences systematic review on treatment for AOS will be discussed along with more recent work. The course will provide demonstrations and detailed discussion of the rationale and procedures for those approaches with the strongest evidence to support clinical use, so that participants can immediately begin implementing these techniques in the clinic.

The articles focused on the published literature that discussed the current state of understanding of the impact of swallowing exercises in the following patient populations: post-stroke, neurodegenerative diseases, and head and neck cancers. Further, the evidence on the impact of swallowing therapy in the pediatric patient population is detailed as well. The first article, by Gary McCullough, explored the literature as it pertains to physiological impairments in swallowing disorders resulting from strokes. In the second article, Michelle Troche and Avinash Mishra discussed the literature’s findings as they relate to strength-based and skill-based dysphagia treatment and treatment strategies in patients with neurodegenerative diseases. The modern management of dysphagia in the head and neck cancer patient population is elaborated in the third article, written by Heather Starmer. In the fourth article, Memorie Gosa and Pamela Dodrill presented the process of effective diagnosis and management of oral-pharyngeal dysphagia in children. Lastly, Lori Morgan Burkhead comprehensively highlighted recent work in dysphagia rehabilitation utilizing the basic principles of exercise therapy as they relate to the interpretation of the content in the aforementioned four articles.

The articles focused on the research that concerns current consensus on various factors associated in caring for the oncology patients, specifically to the function of swallowing and the disorders of swallowing. Antonia Kilcommons and Debra Rawlinson initiated an engaging discussion in regards to swallowing disorders and the complications of long- term feeding in the pediatric population with brain tumors. Cathy Lazarus, on the other hand, elaborated on the cumulative impact of normal age related changes in the swallow mechanism on dysphagia associated with cancer treatments in the elderly population. Pippa Hales and Corine Mossey-Gaston discussed the anatomical and physiological impact of lung surgery on swallowing biomechanics and provided the clinician with treatment and referral considerations. Further, Lisa Evangelista and James Coyle reviewed in-depth the clinical aspects of esophagectomy in the management of esophageal cancers and facilitate understanding of the typical expectations post- surgery. Lastly, practicing clinicians will relate to Andrea Croom’s detailed presentation of the association of different personality-types and patients’ abilities to adhere to multidisciplinary cancer treatment.

This Perspectives focused on cognition and hearing abilities in adults. We shared how working memory capacity and attention should be considered when selecting and programming amplification and providing aural rehabilitation for individuals with hearing loss. Individuals with higher working memory capacity are able to utilize fast-acting compression settings to aid in speech understanding in complex listening environments, while those with lower working memory can experience negative repercussions from such settings. Additionally, clinicians can counsel on cognitive effects on speech understanding and involve top-down strategies in their auditory training. We also examined the effects of aging on psychoacoustic performance, noting an association between cognition and discrimination abilities in older adults. Factors like impaired temporal processing and reduced frequency selectivity impact hearing abilities of adult clients in the clinic, but are rarely assessed in a standard hearing evaluation. Finally, we examine why pure-tone threshold testing is insufficient at predicting hearing handicap in the adults with minimal to mild hearing loss, and how to assess which of these individuals would benefit from rehabilitation and/or amplification. Factors such as previous noise exposure, age, working memory, ability to use conversational context, and speed of auditory processing are important to consider.

Differentiating apraxia of speech (AOS) from other acquired speech and language impairments is important clinically as it provides a framework for understanding the signs and symptoms of the disorder, which in turn influences which specific treatment method will be applied. However, diagnosis remains challenging since it hinges on expert-based judgment of speech features, with minimal guidance on the frequency, severity, or clustering of features required for positive diagnosis. Acquired apraxia of speech (AOS) typically occurs alongside aphasia. Clinicians are often challenged with questions such as, “What should I work on first, and why/how?” and “How can I deliver intensive therapy in today’s healthcare system?” While there are a number of treatment approaches available for AOS, there are very few large-scale investigations of treatment efficacy to inform practice and limited studies that tackle the issues around intensity and concomitant disorders. This course summarizes the pros, cons, and similarities of impairment-based approaches to treatment for individuals with AOS plus aphasia. The outcomes from our latest Academy of Neurologic Communication Disorders and Sciences systematic review on treatment for AOS will be discussed along with more recent work. The course will provide demonstrations and detailed discussion of the rationale and procedures for those approaches with the strongest evidence to support clinical use, so that participants can immediately begin implementing these techniques in the clinic.

These three research articles span a wide range of topics in fluency disorders. First, evidence is presented that supports theories that people who stutter may exhibit increased speech monitoring when compared to typically fluent peers. But studies on PEG tubes have raised doubts about their long-term benefit as well as concerns about their overuse for providing artificial enteral nutrition. This course presents evidence-based information about dysphagia, aspiration, aspiration pneumonia, nutrition support, and the role of PEG tubes in caring for cognitively impaired elders. In addition to using this information to manage care for cognitively impaired patients, practitioners can extrapolate to other patient groups, such as stroke patients, for which the evidence base is inadequate.

Over the years, cochlear implant candidacy has changed dramatically. When devices were first introduced, only patients with little to no ability to recognize speech were considered. Advances in technology and evolving ideas about who can benefit from a cochlear implant have expanded the candidate pool, which now can include adults and children with greater preoperative residual hearing, nontraditional candidates (such as those with additional disabilities), and “off label” patients who do not meet the approved indications for the device but could benefit nonetheless (such as those with single sided deafness or asymmetrical, progressive hearing loss). This course will explain the candidacy changes in greater detail and provide guidelines to help professionals determine if a referral to a cochlear implant center is appropriate for a particular patient. The webinar will outline the roles of audiologists and SLPs in making these recommendations and will also discuss how to counsel patients regarding their possible candidacy for a cochlear implant.
